Follow these simple steps to verify your school's directory information:

1. Go to https://search.finalforms.com.

2. Search for your school.

3. Click the “Login” button under the Staff icon.

4. Click the Calendar icon.

5. Select the current school year.

6. Select the Current School Year Annual Checklist → District-Wide

7. Go to the "Verify Sports" form.

8. Click the "Edit" button in any Sport's row.

9. Check the following information:

   • Gender

   • Sport offerings

   • Season

   • Teams/Levels (Varsity, JV, Freshman, Modified, Unified)

   • Assigned coaches

10. Click Update Sport to save your sport-specific changes.

11. Sign the Verify Sports Form.

12. Click Submit Form to save all of your changes.
