Welcome to the Participation Survey Guide! FinalForms is committed to making annual participation data collection simple and efficient.

1. Log in to your FinalForms account

   1. (For login assistance, click here)

2. Select the “Submit 2025–26 NFHS Participation Survey” button at the top right of your screen

3. Enter your state-wide participation data in the form provided

4. Click “Submit Official Report to NFHS” at the bottom of the page

NOTES:

• You may Save Draft to save your progress and finish at a later date.

• Your sanctioned vs. non-sanctioned sports and activities may not be fully accurate. That's fine for NFHS submission. If you'd like FinalForms to align this with your official designations, please send the list to matt@finalforms.com.

You can submit and re-submit your Participation Survey at any time before the deadline date of June 1st. Rest assured, the most recent submission will be used once entries are closed.
