The purpose of this guide is to help you verify your school’s activity offerings before submitting the survey. Accurate information is crucial for effective communication and planning.

Follow these simple steps to verify your school's directory information:

1. Go to https://search.finalforms.com.

2. Search for your school.

3. Click the “Login” button under the Staff icon.

4. Click the Calendar icon.

5. Select the current school year.

6. Select the Current School Year Annual Checklist → District-Wide

7. Go to the "Verify Activities" form.

8. Click the "Edit" button in any Activity's row.

9. Check the following information:

   • Gender

   • Activity offerings

   • Season

   • Teams/Levels (Varsity, JV, Freshman, Modified, Unified)

   • Assigned advisors

10. Click Update Sport to save your activity-specific changes.

11. Sign the Verify Activities Form.

12. Click Submit Form to save all of your changes.
