Follow these easy steps to check the Participation Survey Submission Status:

1. Go to https://search.finalforms.com.

2. Search for your state association.

3. Click the “Login” button under the Administrators icon.

4. Navigate to MANAGE > SCHOOLS.

5. Locate the "By Name" area, enter the school name, and press the Return key on your keyboard.

6. Click the FILTER button (Looking Glass icon).

7. Locate the Participation Survey section in the filter panel.

8. Select one of the following options:

   • Submitted - to view schools that have completed the survey.

   • Not Submitted - to view schools that still need to complete the survey.

Once you have made your selection, the school will remain visible under one of those options.
